WORKSHOP:  The employer had not conducted an annual and/or periodic review of the hazardous energy control procedures in the machine specific, such as (but not limited to) the printing press, binder, perfect binder, and saddle stitcher trimmer.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.    WORKSHOP:  The employer had not ensured through written certificatiion that each operator of the powered industrial truck (PIT) was appropraitely trained and evaluated.
