29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a)West side of building at 1725 Springfield Ave., Maplewood, NJ:Employer allowed his employee to work from a scaffold in excess of 18 feet from the ground below with no fall protection system in place, exposing him to fall hazards.
