29 CFR 1910.212(a)(3)(ii):    Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a)  Open Hearth Woodyard - On or about 11-26-12 - Employees and therapy work clients were exposed to amutation hazards from an unguarded wood cutter. Employees and therapy clients were able to reach in between the splitter head and the piece of wood being pushed by the ram.
