This condition exposed the employees to an amputation hazard. (b)Stacy Machine & Tooling, Inc. at 2810 Industrial Ln., Broomfield, CO 80020: Machine guarding was not provided in that the Shark and Bridgeport mills were not fitted with a guard to protect the employee from rotating parts. This condition exposed the employees to an amputation hazard. (c)Stacy Machine & Tooling, Inc. at 2810 Industrial Ln., Broomfield, CO 80020: Machine guarding was not provided in that the Maximat lathe was not fitted with a guard to protect the employee from the rotating chuck. This condition exposed the employees to an amputation hazard.
