29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:       (a)Stacy Machine & Tooling, Inc. at 2810 Industrial Ln., Broomfield, CO 80020:   The employer did not ensure that each affected employee used appropriate eye or face protection when potentially exposed to flying particles in that employees used non-safety prescription glasses when working with metal working tools and materials.  This condition exposed the employees to eye injury.

29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operators employees from hazards created by ingoing nip points, rotating parts, flying chips and sparks:    (a)Stacy Machine & Tooling, Inc. at 2810 Industrial Ln., Broomfield, CO 80020:  Machine guarding was not provided in that the Roll In steel cutting bandsaw was not fitted with a guard to protect the unused portion of the blade.  This condition exposed the employees to an amputation hazard.    (b)Stacy Machine & Tooling, Inc. at 2810 Industrial Ln., Broomfield, CO 80020:  Machine guarding was not   provided in that the Shark and Bridgeport mills were not fitted with a guard to protect the employee from rotating parts.  This condition exposed the employees to an amputation hazard.    (c)Stacy Machine & Tooling, Inc. at 2810 Industrial Ln., Broomfield, CO 80020:  Machine guarding was not   provided in that the Maximat lathe was not fitted with a guard to protect the employee from the rotating chuck.  This condition exposed the employees to an amputation hazard.

29 CFR 1910.215(a)(4):  Grinding machinery was used without a work rest to support offhand grinding work:    (a)Stacy Machine & Tooling Inc., at 2810 Industrial Ln., Broomfield, CO 80020:  The left grinding wheel on the Ryobi 8" bench grinder used for offhand grinding did not have a work rest.  This condition exposed the employee to the hazard of the work being jammed between the wheel and the wheel cover.
