OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards: On or about November 6, 2012, an employee worked on a service rig tubing board during the completion of an oil/gas well removing and inserting steel pipe tubing into the well. The anchorage for the secondary egrees line on the service rig derrick was not adequate where a pair of driven stakes were only driven into the ground approximatley 18 inches deep and had not be verified to meet American Petroleum Institute (API) RP 54, paragraph 6.10 and the manufacturer's instructions.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: On or about November 6, 2012, five employees operating a service rig during the completion of an oil/gas well were removing and laying down steel pipe. Four of the employees were not wearing protective hard hats exposing them to the hazard of being struck on the head by falling pipe.
