29 CFR 1926.451(f)(7):  Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person:      Untrained employees erected a fabricated frame scaffold 4 bucks high on a porch roof with an eave of 18 feet under the direction of a supervisor with no scaffold training.

29 CFR 1926.1053(b)(16):  Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:     In order to perform remodeling work, employees used an extension ladder to access the flat roof of a porch at 18 feet   The ladder had one defective foot causing the ladder to lean.
