29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means.    a) Jobsite: 1845 Fitzwater St, Philadelphia, PA - On or about 11/26/12, the supported tubular welded frame scaffold, measuring approximately 33 feet in elevation and 5 feet in width, was not restrained from tipping or falling over by guying, tying, bracing, or equivalent means. Employees were performing masonry operations from the 4th working platform approximately 27 feet above ground level, and from the outrigger section, approximately 30 feet above ground level.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:      a) Jobsite: 1845 Fitzwater St, Philadelphia, PA - On or about 11/26/12, the supported tubular welded frame scaffold was at risk of collapsing or shifting because the legs did not bear on an adequate firm foundation, but on planks  2 and 3 feet above the window wells. Employees were performing masonry operations from the 4th working platform approximately 27 feet above ground level, and from the outrigger section, approximately 30 feet above ground level.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) Jobsite: 1845 Fitzwater St, Philadelphia, PA - On or about 11/26/12, employees were not provided with a fall protection system while working from the outrigger and platform sections of the supported tubular welded frame scaffold.  Employees performing masonry operations from the outrigger section of the scaffold were exposed to fall hazards approximately 30 feet above ground level, and approximately 27 feet above ground level from the 4th working platform.
