29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    The employer allows employees to be exposed to a fall hazard created by performing work activity from the roof of the residential structure without properly installing fall protection systems.  The most recent occurrence was at 1816 Pheasant Run Drive, Maryland Heights, MO 63043, where workers were observed on top of the 6:12 pitch roof, approximately 25 feet above ground, working to install the gutter system without any form of fall protection.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to upper level landing surfaces and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    Two employees gained access and egress by utilizing versatile portable ladders that were positioned at or below the eave height and did not extend at least 36 above the roof landing.  The Werner Multi-Purpose ladder was improperly positioned at the front of the residential structure and the Little Giant ladder system was improperly positioned on the garage roof for access to the south side of the second story roof deck.  The fall hazard measured approximately 8 feet, 8 inches at the porch and approximately 5 feet to the garage roof deck.

29 CFR 1926.1053(b)(4):     Ladders were used for purposes other than the purposes for which they were designed:    The workers utilized two ladders in the A-frame configuration to access the porch and second story roof decks.  The Werner ladder was positioned at the front walkway for porch roof access and the Little Giant versatile ladder was positioned on the garage roof leading to the second story roof.  The workers were exposed to a fall hazard as they had to climb on the top two rungs of each step ladder to step off onto the roof deck.
