29 CFR 1926.302(a)(1):     The noncurrent-carrying metal parts of electrical power operated hand tool(s) were not grounded, or of the approved double insulated type    Bus Terminal site:  On or about November 27, 2012 and times prior thereto, a Craftsman Skill saw S/N 885206469 had the ground pin missing.  This exposed employees to shock hazards from using a power tool that was not grounded.

29 CFR 1926.501(b)(2)(ii):     Each employee on a walking/working surface 6 feet (1.8 m) or more above a lower level where leading edges are under construction, but who was not engaged in the leading edge work, was not protected from falling by a guardrail system, safety net system, or personal fall arrest system.Note:  If a guardrail system was chosen to provide the fall protection, and a controlled access zone had already been established for leading edge work, the control line may be used in lieu of a guardrail along the edge that parallels the leading edge.    Bus Terminal building:  On or about November 27, 2012 and times prior thereto, two employees were installing metal roof materials.  This exposed employees to a fourteen foot fall  hazard off the roof.

29 CFR 1926.1052(c)(1):     Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge.    Bus termial building:  On or about November 27, 2012 and times prior thereto, a stairway with 9 risers did not have a temporary hand rail installed.  This exposed employees to a fall hazard down the stairs in the event they tripped.

29 CFR 1926.405(g)(2)(iv):     Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    Bus terminal site:  On or about November 27, 2012 and times prior thereto, a flexible cord in use did not have proper strain relief.  This exposed employees to shock hazards if it was used on a non GFCI system.

This exposed employees to struck-by hazards in the event they did not see the vehicle backing up.
