29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) In the processing room, line #2 conveyor has an ingoing nip point at the top of the belt approximately 4" from the frame of the conveyor. The water valve is approximately 12" from nip point. b) Surge tank has pinch points on line #1 created by 13" stroke cylinders on discharge doors. Approximately 3" of clearance between agitator and arm for door. c) Bowl chopper #1, outfeed to conveyor, approximately 6" to nip point from cylinder frame. d) Packaging room conveyor ingoing nip point approximately 3" from frame. e) Loaf line box machine pinch point exposed by broken guard piece missing approximately 3-1/2" x 7". ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED
