B. Employees were observed walking near window openings which was unguarded at the edge to the outside ground level below, exposing employees to falling distances of greater than 6 feet.

The stairway was not equipped with at least one handrail and one stairrail system along the unprotected edge, exposing the employees to a falling distance of 9 3/4 feet.
