29 CFR 1910.134(c)(2)(i):  Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:      a.) The Seneca Seneca Hardwood Lumber Company, Inc., 212 Seneca Hardwood Road, Cranberry, PA  16319,  on or about November 29th, 2012, and times prior thereto:  Company employees voluntarily used air purifying half mask respirators (Moldex brand 8000 series with Moldex 8990 P100 cartridges) during cleanup operations throughout the facility.  Employees that used the half mask respirators were not provided the information in Appendix D to this section.

The Seneca Hardwood Lumber Company, Inc., 212 Seneca Hardwood Road, Cranberry, PA  16319,  on or about November 29th, 2012, and times prior thereto:  Company employees voluntarily used air purifying half mask respirators (Moldex brand 8000 series with Moldex 8990 P100 cartridges) during housekeeping operations throughout the facility.  The company did not establish portions of a Respiratory Protection Program that covered Medical Evaluations and Respirator Maintenance and Care.

29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program:    a.)The Seneca Seneca Hardwood Lumber Company, Inc., - Production Areas - 212 Seneca Hardwood Road, Cranberry, PA  16319, on or about November 29th, 2012, Quest Technology NoisePro dosimeters were placed on the Flooring/Molding saw operator Ripping Saw operator, and Planing Saw operator for totals of 395, 384, 363 minutes, respectively. The equivalent 8 hour dose and Time Weighted Average (TWA) for the Planing Saw Operator was 73.5% and 87.7 dBA.  The Company did not provided annual hearing conservation training for affected employees.
