29 CFR 1926.150(c)(1)(vi):A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas are being used on the jobsite:    (a) 93 Woodstock St Springfield,MA:On or about November 9, 2012 no fire extinguisher was provided for this job site where gasoline was present in a red plastic gas container and gas powered tools were used in violation of this subpart.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) 93 Woodstock St Springfield, MA: On or about November 9, 2012 an employee was walking on a ladder (in a horizontal position) used to bridge the chasm between the front lawn and the garbage container in the driveway of this residence which had unprotected sides more than 6.5 feet above the ground below.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) 93 Woodstock St Springfield, MA: On or about November 9, 2012, employees were observed performing roofing work on this residence between 9.7 and 14.6 feet above the ground below without any fall protection:    Fourth Generation Construction,Inc was previously cited for a  violation of this occupational health and safety standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 315515106 which became final order on November  4, 2011 with respect to a violation at 223-225 Hampden St  in Chicopee, MA:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
