29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    On November 27, 2012 at a worksite located at 284 S. York Road Elmhurst IL, an employee was working from a ladder jack scaffold and was not wearing a personal fall arrest system. The employee was exposed to a fall hazard of approximately 14.9 feet.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903(d).

29 CFR 1926.452(k)(5):     Scaffold platforms were bridged on to another.    On November 27, 2012 at a worksite located at 284 S. York Road Elmhurst IL, an employee was working from a ladder jack scaffold that the platform was resting on a plank from the home to the rungs on the ladder. The employee was exposed to a system collapse of the ladder jack scaffold.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903(d).

29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On November 27, 2012 at a worksite located at 284 S. York Road Elmhurst IL, an employee was working on a roof on the south west corner of the home and was not utilizing any means of fall protection. The employee was exposed to a fall hazard of approximately 10.4 feet.       Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903(c).
