(b) Worksite, second story roof - An employee was performing roofing work along the edges of a steep pitch roof (8:12), without fall protection. The employees was exposed to falling approximately 14 feet to the next lower level and/or 28 feet to the ground below.The employee was not protected from the fall hazards by a guardrail, safety net, personal fall arrest system, or alternate fall protection measure, on or about November 30, 2012.    (c) Worksite, first story roof - Employees were performing roofing work on a low pitch roof (4:12), without fall protection. The employees were exposed to falling approximately 11 feet to the ground below. The employees were not protected from the fall hazards by a guardrail, safety net, personal fall arrest system, or alternate fall protection measure, on or about November 30, 2012.        Note: In addition to abatement certification, the employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Worksite, west side of home, first story roof - Employees used a 24 foot fiberglass extension ladder, Werner Model D6324-2, to ascend and descend from the first story roof. The ladder's side rails were even with the eve of the roof and did not extend beyond the landing being accessed, on or about November 30, 2012.    (b) Worksite, werst side of home, second story roof - Employees used a 36 foot aluminum extension ladder, Manufacturer and model unknown, to ascend and descend from the second story roof. The ladder's side rails extended beyond the landing being accessed approximately 18 inches, on November 30, 2012.      In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
