29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) At the A1/A2 Furnace - On or about November 16, 2012, the employer required the use of a 3M 2-strap dust mask respirator, and had not established and implemented a written respiratory protection program, including, but not limited to a  medical fitness evaluation; annual respiratory protection fit testing and training in the proper use and limitations, care for, cleaning, disinfecting, and storage of, respiratory hazards requiring respirator use, and possible health hazards involved in using respiratory protective equipment for employees, cleaning the A1/A2 furnace roller.    b) At the Vertical Furnace - On or about November 16, 2012, the employer required the use of a tight-fitting full-face respirator with dust cartridges, and had not established and implemented a written respiratory protection program, including, but not limited to a  medical fitness evaluation; annual respiratory protection fit testing and training in the proper use and limitations, care for, cleaning, disinfecting, and storage of, respiratory hazards requiring respirator use, and possible health hazards involved in using respiratory protective equipment for employees, cleaning the vertical furnace rollers.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) On the Production Floor and in the Warehouse - On or about November 16, 2012, the employer had not implemented a written hazard communication program which at least described how the criteria specified in paragraphs (f) Labels, (g) Material Safety Data Sheets, and (h) Training was being met for employees exposed to chemicals such as liquid nitrogen used in furnaces as shield gas, liquid hydrogen used in the facility space heaters to heat the workspace, propane used to fuel the forklifts throughout the facility, and the grinding oil used to lubricate the rollers while they were resurfaced.
