Violation observed on or about December 3, 2012    The Great American Roofing Company, LLC was previously cited for a violation of the Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection Number 282461, Citation Number 2, Item Number 2, Issued on May 04, 2012, and became final order on May 25, 2012, with respect to a workplace located at 638 Washington Avenue, Washington Township, NJ 07676.    The Great American Roofing Company, LLC was previously cited for a violation of the Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection Number 314678020, Citation Number 1, Item Number 2, Issued on June 08, 2011, and became final order on June 29, 2011, with respect to a workplace located at 110 Chestnut Ridge Road, #337, Montvale, NJ 07645. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     a. 3 Gilbert Place, West Orange, NJ     Employees used aluminum ladders to access the roof that did not extend at least three feet above the upper level landing surface and ladders were not secured at the top, thereby exposing employees to fall hazards.    Violation observed on or about December 3, 2012    The Great American Roofing Company, LLC was previously cited for a violation of the Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA Inspection Number 313982886, Citation Number 1, Item Number 3, Issued on October 27, 2010, and became final order on November 17, 2010, with respect to a workplace located at 199 Evergreen Road, Edison, NJ 08817.
