29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. a. At the site, there were two employees installing exterior sheathing from a basket that was attached to the forks of a rough terrain forklift. The rough terrain forklift would move horizontally while the employees were in the forklift's basket, thereby exposing the employees to struck-by and/or caught-between hazards.

29 CFR 1926.602(c)(1)(ii): No modifications or additions which affect the capacity or safe operation of the equipment shall be made without the manufacturer's written approval. If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. In no case shall the original safety factor of the equipment be reduced. a. At the site, there were two employees installing exterior sheathing from a basket that was attached to the forks of a rough terrain forklift. The basket that the employees were working out of was not approved for use by the manufacturer, thereby exposing the employees to a fall, struck-by and/or caught-between hazard.
