29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was   6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      On the west side overhang of an existing Citgo Service Station under renovation located at 4900 North Federal Highway   in Lighthouse Point, Florida:  Employees installing a new Food Mart sign on an existing Citgo Service Station while on   a walking/working surface approximately 11 ft. 1 inch above a lower level with unprotected edges were not protected from falling to that lower level, on or about December 3, 2012.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On site of an existing Citgo Service Station under renovation located at 4900 North Federal Highway in Lighthouse Point, Florida:  The employer did not provide a training program for employees performing work installing a new Food Mart sign on a walking/working surface while exposed to potential fall hazards that would enable them to recognize the hazards of falling, on or about December 3, 2012.     PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On the south side overhang of an existing Citgo Service Station under renovation located at 4900 North Federal Highway in Lighthouse Point, Florida:  Employees gaining access to and from a walking/working surface to install a new Food Mart sign 11 ft. 1 inch above a lower level using a 16 ft. aluminum extension ladder did not have the side rails of that ladder extended at least 3 ft. past the upper landing surface to which they were gaining access to, on or about December 3, 2012.

29 CFR 1926.1053(b)(8):  Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    On the south side overhang of an existing Citgo Service Station under renovation located at 4900 North Federal Highway   in Lighthouse Point, Florida:  Employees gaining access to and from a walking/working surface installing a new Food Mart sign approximately 11 ft. 1 inch above a lower level using an aluminum extension ladder set up in an active parking lot did not have that ladder secured to prevent accidental displacement, on or about December 3, 2012.
