29 CFR 1910.95(i)(2)(ii): The employer did not ensure that hearing protectors were worn by any employee who was exposed to an 8-hour time-weighted average of 85 decibels or greater:    (a) Cast Rite Metal Company - The employer did not ensure that the Knock-Off employee (exposed to 92.6 dBA) was wearing hearing protectors, on or about December 14, 2012.    (b) Cast Rite Metal Company - The employer did not ensure that the Saw Operator (exposed to 97.6 dBA) was wearing hearing protectors, on or about December 14, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by the unused portion of the band saw blade:    (a) Cast Rite Metals Company, Inc. - The employee operating the Du-All Band Saw was exposed to a digital amputation hazard from the approximately seven (7) inches of unguarded, unused band saw blade, on or about December 14, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1026(d)(1): The employer who had a workplace or work operation covered by this section did not determine the 8-hour TWA exposure for each employee exposed to chromium (VI):    (a) Cast Rite Aluminum Company, Inc. - The employer did not conduct an initial employee exposure assessment for the employees melting and pouring the Tenzalloy 700 Series alloy which contained chromium, on or about December 3, 2012.    No abatement certification required.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:    (a) Cast Rite Metal company, Inc. - The employer did not develop a monitoring program for the Belt Grinder, Saw Operator, Bench Grinder, Molder, or Knock-Off Operator. All of which were found exposed to occupational noise greater than an eight hour time weighted average 90.0 decibels, on or about December 14, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
