29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedure: Authorized lockout employees were not retrained on procedures for deenergization of the hydraulic energy source for CNC Mill N-109 when it was introduced into the assembly area and did not know that the hydraulic power for the bearing presses was lost at the same time that the hydraulic power was shut-off at the ceiling for CNC N-109. *Abatement documentation is required per 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded. The guarding device was shall be in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: The assembly bearing presses were not designed so that in the event of a hydraulic power failure, the restoration of hydraulic power would not result in any hazardous cycling of the ram. *Abatement documentation is required per 29 CFR 1903.19(d).
