29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about December 5, 2012, employees drilling out post tensionsing cables were not protected from a 16-foot fall hazard by use of a fall protection system.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a. On or about December 5, 2012, on the east side of the building, a ladder was not provided to access the man basket, exposing employees to a 6-foot fall hazard.
