OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards:    (a)  Location - Warehouse:  On or about 12/18/12 multiple storage racks, holding pallets of mayonaisse and/or dressing product with an average weight of 1850 pounds each, had structural damage exposing employees to hazards of being struck or crushed by falling pallets and boxes.

29 CFR 1910.22(d)(1):  On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate:    (a)  Location -  Main Building, Loading Dock  On or about 12/5/2012, the load rating for the storage racks located above the loading dock overhead doors was not posted.  The storage racks held pallets of rolled labels.    (b)  Location - Warehouse  On or about 12/18/2012, the load rating for the storage racks was not posted. The storage racks held pallets of mayonaisse and dressings in boxes.

29 CFR 1910.303(c)(3)(i):  Conductors shall be spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices shall first be spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors shall be covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:    (a)  Location - Production Area  On or about 12/5/12 an electrical cord, servicing the boxing machine, was repaired with black electrical tape.    (b)  Location - Forklift Charging Area  On or about 12/5/12 an electrical cord, servicing a powered industrial truck charger, was repaired with yellow electrical tape.    (c)  Location - Forklift Charging Area  On or about 12/5/12 an electrical cord, servicing a powered industrial truck charger, was repaired with black electrical tape.
