29 CFR 1910.147(f)(3)(i):     A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft:    a)	Miller Haller, south side, on the Mark Andy 10 inch, 6 color 4150 press; the press was locked out during maintenance and setup, multiple employees were working on the machine.  The hazardous energy sources for the machine were locked out with only one lock and one tag, controlled by only one employee.  All employees exposed to the hazard of unexpected release of hazardous energy did not have a lock on the energy sources by applying their own personnel lock on a  hasp that accepts multiple locks, or by using  a lock box that accepted multiple locks to control the keys to the individual locks applied on the machine.
