29 CFR 1910.37(b)(4):     If the direction of travel to the exit or exit discharge is not immediately apparent, signs must be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge. Additionally, the line-of-sight to an exit sign must clearly be visible at all times.    a. At the Digico Imaging facility located in Columbus, Ohio the employer had a doorway on the South side of the finishing room that led to a hallway that was not identified as an exit access.

29 CFR 1910.178(l)(6):     Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.    a. At the Digico Imaging facility located in Columbus, Ohio the employer did not ensure that all employees received formal training and evaluation before being allowed to operate a forklift.

29 CFR 1910.303(b)(2):     Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.    a. At the Digico Imaging facility located in Columbus, Ohio the employer had wired a pendant electrical outlet that was installed with a 4 inch receptacle box that was manufactured with the knock outs.  The 4 inch box was not listed and labeled to be used as a pendant receptacle.

29 CFR 1910.132(d)(2):     The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a. At the Digico Imaging facility located in Columbus, Ohio the employer had employees that were required to wear PPE such as safety glasses, but the employer had not performed the written certification of what PPE was required to control which hazards.
