29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.    Worksite: The employee/foreman who was accessing the ladder jack scaffold platform from the ladder that was used to support the platform was not provided with a safe means of access to the platform.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.    Worksite: The employee/foreman who was working from the ladder jack scaffold platform at a height of approximately 19 feet above the ground was not protected from falling by any means.

Worksite: The employees who were working on the steep pitch roof with an eave height of approximately 22 feet above the ground were not protected from fall hazards by the use of guardrails, safety nets or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    Worksite: The extension ladder that was used to gain access to the lower roof at a height of approximately 8 feet above the ground did not have its side rails extended 3 feet above the landing surface.
