29 CFR 1910.243(c)(1):   Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a):  On or about November 30, 2012, approximately six portable angle grinders were used without guarding, exposing employees the hazards of being caught in the spinning wheel or struck by debris of an exploding wheel.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that grinders are used with their guarding is in place.

29 CFR 1910.303(b)(2):   Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about November 30, 2012, equipment in the facility was not used in accordance with instructions included in the listing or labeling, exposing employees to a shock hazard.   a: Non-metallic sheathed cable was found powering a receptacle box along the south wall of the facility.  The box was used to power a spotlight.  b: Non-metallic sheathed cable not in conduit was used to power a spotlight along the south wall of the facility.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that electrical equipment and wiring is used as listed and labeled.

29 CFR 1910.305(g)(1)(iv)(A):   Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  On or about November 30, 2012, a spotlight in the northeast corner of the facility was powered with an extension cord as permanent wiring, exposing employees to shock and burn hazards.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that flexible cords are not used as permanent wiring.
