29 CFR 1917.119(e)(1)(i):  The employer did not maintain the portable ladder in safe condition and was not removed from service due to broken, split or missing rungs, cleats or steps:    a) Jobsite, 1000 Madison Avenue, West Elizabeth, PA:  On or about November 30, 2012, an employee did not inspect the ladder and used a section of a portable extension ladder that was missing the cleats.  It was used to get from the barge dock to the lower barge for the employee to weigh the loaded barge.

29 CFR 1917.119(f)(9):  Ladders were not fitted with slip resistant bases and secured at top or bottom to prevent the ladder from slipping:    a) Jobsite, 1000 Madison Avenue, West Elizabeth, PA:  On or about November 30, 2012, an employee was using a 12 foot section of an aluminum extension ladder that was missing its slip resistant bases (cleats).  The employee was using this ladder to access a barge from the barge dock.
