29 CFR 1926.405(b)(1):  Conductors entering boxes, cabinets, and fittings were not protected from abrasion:      (a) On or about November 2, 2012, at a job site located at 415 S. Golden Ave. in Springfield, MO, workers were exposed to possible electrical shock hazards while working with and around a circular box fan that had electrical conductors exposed to being cut or abraded by sharp edges on an electrical box mounted on the fan.

29 CFR 1926.300(b)(5):  Fan blade guards had openings larger than one-half inch (1.27 cm):      (a) On or about November 2, 2012, at a job site located at 415 S. Golden Ave. in Springfield, MO, workers were exposed to possible finger injuries while working with and around a circular box fan that was missing the center "plug" on one side of its guarding cage creating an opening of approximately 4 inches.
