29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      The employer failed to develop energy control procedures that include specific procedures, training and periodic inspections. Control of hazardous energy procedures should include but should not be limited to the following equipment: CNC machines and Injection Molding Machines.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Employees are exposed to cuts lacerations, fractures and amputations while operating the following equipment. Haas CNC Machine, model VF-3 serial number 17660. Safety devices were disabled to perform production activities.        Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
