Employees were exposed to crush and struck-by hazards:    a) Facility - The Allis-Chalmers LPG-powered forklift did not have a seatbelt installed to restrain workers inside the cabin in case of tip-over.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Plant - Employees performed cleaning, maintenance, and repairs of machinery including (but not limited to) the Federal punch machines, the DoAll band saw, the Komatsu shear, the cutting oil recovery machine, the Lincoln robotic welder, the New Britain bar machines, the Fadal CNC milling machines, and the Acme-Gridley revolver lathes. The employer did not develop, implement, and utilize individual written lockout tagout procedures.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.303(b)(1):     Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    a) Plant - On or about 11/28/12 the junction box in the loading area of the Acme-Gridley revolver lathe was broken. Part of the wall on the right side was missing and the internal conductors were exposed.      b) Plant - On or about 11/28/12 the metallic spiral conduit entering the junction box in the loading area of the Acme-Gridley revolver lathe was broken.  The internal conductors were exposed and subjected to strain.    c) Plant, Cutting oil recovery station - On or about 11/28/12 the flexible cord powering the Dayton 1/2 Ton electric crane was damaged. The outer jacket was broken and covered with electrical tape.    d) Plant, Welding Station #2 - On or about 11/28/12 the flexible power cord for the cooling fan (mounted on the steel column) was damaged.  The outer jacket was broken in proximity of the switch, and the internal conductors were exposed.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The cable passed inside the cage and was not protected from the sharp metallic edges of the fan box.
