29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the following machine stations, employees were not protected from being struck by the moving parts of the large, rotational mold fixtures. Two employees worked together at each machine station. One employee operated the controls to move the mold fixture arm from one position to another, and was protected only by the distance required for his position at the control panel. A second employee worked on the mold fixture while standing on an elevated mobile ladder stand or mobile work platform and was not protected from being struck by the moving parts. a. #282 Rotational Mold Machine (Manufacturer: Ferry Industries, Inc.) b. #430 Rotational Mold Machine (Manufacturer: Ferry Industries, Inc.) 29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. a. #330 Rotational Mold Machine Area. An electrical box (110v) covering the conductors for the machine's wall-mounted control switch had an opening in the box, due to a missing knockout. There is no abatement documentation or certification required for this item.
