29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  On or about December 6, 2012, employee working on aerial lift fixing an electric sign at I-20 exit 133 in Midland, TX, was not wearing any type of fall protection equipment. This condition exposed employee to falls and struck-by hazards.
