29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:    (a) Warehouse Roof located at 4620 South Atlantic Road, Smyrna, GA 30080: Employee using a 20 pound propane tank with torch attachment was engaged in torch-down roofing work exposed to burn hazards when a fire extinguisher was not provided on the roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    (a) Warehouse Roof located at 4620 South Atlantic Road, Smyrna, GA 30080: Employees were exposed to fall hazards of approximately 28' to the concrete surface below while engaged in roofing activities on a flat warehouse roof with no fall protection system in place.
