29 CFR 1926.451(e)(1):     Scaffold platforms were more than 2 feet (0.6 m) above or below a point of access and portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not available to be used.    On or about 11/30/2013, employees engaged in bricklaying activities at 1601 South Jefferson, St. Louis, MO 63104 were utilizing a scaffold which did not have any means of sae access.  Employees had to climb on the saffold framework itself, which was not designed for that purpose.

On or about 11/30/2013, employees engaged in bricklaying activities at 1601 South Jefferson, St. Louis, MO 63104 were utilizing a six foot step ladder to access a scaffold which was 68" above the ground.   Step ladder was used in folded position, not opened and in locked position.
