29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.      On or about December 11, 2012, on the north side, an employee was exposed to a fall hazard of 15 feet while changing out lamps on a bill board from a vehicle-mounted and rotating work platform.

29 CFR 1910.132(f)(3)(i): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (f)(2) of this section, the employer shall retrain each such employee.      On or about December 11, 2012, on the north side, an employee was exposed to a fall hazard of 15 feet while changing out lamps on a bill board from a vehicle-mounted and rotating work platform.  The employer is to provide retraining on fall protection to the employee exposed to the fall hazard.
