29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a)  On or about December 12, 2012 and at times prior thereto at the jobsite; employees were cleaning up shingles from the ground while other employees were actively removing shingles from the roof and throwing them to the ground.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  On or about December 12, 2012 and at times prior thereto at the jobsite; an employee was performing roofing work from a ladder jack scaffold approximately 20' in height with no personal fall arrest system in place.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) On or about December 12, 2012 and at times prior thereto at the worksite; an employee was removing shingles from the peak of an 8 on 12 sloped roof using slide guards as his only type of fall protection.
