The violation occurred in the Phase I engine room where the employer failed to ensure that the 2009 Phase I & II process hazard analysis teams recommendations were resolved in a timely manner and that the resolution(s) were documented.  This condition exposed employees to inhalation hazards.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure that the process hazard analysis teams recommendations are resolved in a timely manner and that the resolution is documented.

The violation occurred in the Phase II cold storage facility where the employer failed to ensure that a pre-startup safety review was completed before the startup of the Phase II ammonia refrigeration system.  This condition exposed employees to inhalation hazards.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure a pre-startup safety review is conducted for new or modified facilities.

The employer does not ensure written procedures are established and implemented to maintain the on-going mechanical integrity of process equipment.    a.  The violation occurred in the Phase I & II engine rooms where the employer failed to ensure detailed written procedures were established and implemented for testing of safety cutouts. Identified cutouts include but are not limited to Compressor Discharge High/Low Pressure, Compressor High/Low Oil Pressure Differential, Compressor High Discharge Temperature, and High/Low Vessel Levels. This condition exposed employees to inhalation hazards.    b.  The violation occurred in the Phase I & II engine rooms where the employer failed to ensure detailed written procedures were established and implemented for testing of engine room safety systems. Identified engine room safety systems include but are not limited to Emergency Ventilation, ammonia detectors & calibration, and the emergency stop (e-stop). This condition exposed employees to inhalation hazards.    c.  The violation occurred in the Phase I & II engine rooms where the employer failed to ensure detailed written procedures were established and implemented for determining locations for conducting thickness measurements of piping and vessels.  This condition exposed employees to inhalation hazards.    d.  The violation occurred in the Phase I & II engine rooms where the employer failed to ensure detailed written procedures were established and implemented for the visual external inspections of insulated piping and vessels.  This condition exposed employees to inhalation hazards.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure written procedures are established and implemented to maintain the on-going mechanical integrity of process equipment.
