29 CFR 1926.416(a): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by deenergizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) Jobsite, Arnold Palmer Motors, Route 30, Latrobe, PA: On or about December, 12, 2012, employees were walking/working in close proximity to electric lines while performing roofing activities and were exposed to electric shock. The employer did not know the voltage of the line and did not ensure that the insulation on it would sufficiently protect the employees.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safee, ty monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Jobsite, Arnold Palmer Motors, Route 30, Latrobe, PA: On or about December 12, 2012, employees were working from from a commercial roof, putting ice and water shield on, and were not protected from falling. The fall hazard from the lower roof was approximately 9 feet and the pitch of the roof was 4 in 12.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Jobsite, Arnold Palmer Motors, Route 30, Latrobe, PA: On or about December 12, 2012, employees were using a 16 foot aluminum extension ladder to access the roof and this ladder only extended about 12 inches above the roof. The fall hazard to the employees was approximately 9 feet to the ground below.
