29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: a) Oustside dust collection area: On or about December 11, 2012, and prior to, the employer exposed employees to fall hazards from not ensuring a portable stair system with more than four risers had a standard rail system consisting of a midrail. b) Assembly area: On or about December 11, 2012, and prior to, the employer exposed employees to fall hazards from not ensuring that two portable stair systems with more than four risers had a standard rail system consisting of a midrail.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein: a) Lamination Area: On or about December 11, 2012, and prior to, the employer exposed employees to hazards of unlabeled container(s) without identification of the hazardous chemical including but not limited to a 5 gallon bucket of acetone on chop sprayer number 2 that has a flashpoint of - 4 degrees fahrenheit and a Lower Explosive Limit of 2.5 percent.
