29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.     a)  Charm Builders, Ltd. on or about December 13, 2012, at the address of 205 West 4th St. Port Clinton, OH: The employer failed to ensure employees were protected from falls while removing old roofing material from a steep sloped commercial type structure. Employees working in this condition were exposed to falls of approximately 20 feet to the ground below.    Charm Builders, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 311841043, citation number 1, item number 1 and was affirmed as a final order on April 11, 2009, with respect to a workplace located at 5571 Autumn Chase Dr. Columbus, OH.    Charm Builders, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314732249, citation number 2, item number 1 and was affirmed as a final order on August 16, 2010, with respect to a workplace located at 12526 Clifton Ave. Lakewood, OH.    Charm Builders, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 315001917, citation number 1, item number 2 and was affirmed as a final order on December 3, 2010, with respect to a workplace located at 3020 W. Market St. Fairlawn, OH.
