29 CFR 1910.22(a)(1):  All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:         (a)  O'Keefe Casting, 2401 E. 28th Street, Lorain, Ohio:  On or about February 28, 2013, the employer did not ensure that that the core making and sand mold preparation areas were kept in an orderly manner.  Extraneous material including combustible cardboard and metallic items were positioned adjacent to exit routes putting employees at risk of serious injury in the event the items fell and struck the employees or impeded employee egress in the event of an emergency.

29 CFR 1910.22(a)(2):  The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition:              (a)  O'Keefe Casting, 2401 E. 28th Street, Lorain, Ohio:  On or about February 28, 2013, the employer did not ensure that that the floors in the shakeout/finishing room were kept clean of accumulated aluminum dust and chips.  The aluminum dust is combustible and employees are at risk of serious injury from fire hazards.

29 CFR 1910.36(g)(2):  An exit access was not at least 28 inches wide at all points.  Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access:           (a)  O'Keefe Casting, 2401 E. 28th Street, Lorain, Ohio:  On or about February 28, 2013, the employer did not ensure that the exit and exit access were at least 28 inches wide.  The exterior door in the core making area was partially blocked by an electrical panel reducing the width to 20 inches.  The walkway leading to the door was not the full width of the exterior door.

29 CFR 1910.215(a)(2):  The safety guard did not cover the spindle end, nut, and flange projections:           (a)  O'Keefe Casting, 2401 E. 28th Street, Lorain, Ohio:  On or about December 13, 2012, the employer did not ensure that that the spindle, nut, and flange on the pedestal grinder were covered.  Employees were exposed to serious injury from the rotating spindle and nut.
