Main St., Houma, LA; the employer failed to ensure an employee wore eye protection while operating a Dewalt screwgun. The employee was exposed to a struck-by hazard.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.      a) 5995 W. Main St., Houma, LA; an employee worked from an elevated personnel platform that was not securely attached to the lift to prevent the platform from sliding off. The platform was used with a Terex TH844C forklift to access a building that metal panels were being installed to the exterior. The employee was approximately 30 feet above the ground.
