Complete inspections of the crane shall be performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section, depending upon its activity, severity of service, and environment, or as specifically indicated below. These inspections shall include the requirements of paragraph (j)(2) of this section and in addition, the following items. Any deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:    On or about Deember 13, 2012 periodic inspections had not been performed on:    a. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed;    On or about December 13, 2012, access to the electrical panel box in the storage area was blocked.  Employees were exposed to electrical hazards.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.      On or about December 13, 2012, in the paint area, employees using a drill with a damaged cord were exposed to electrical shock.
