29 CFR 1910.23(d)(1):   Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section:  a) Main Production hangar - On or about December 18, 2012, employees used portable wood stairs, with more than 4 risers that did not have standard railings installed, to access the interior of helicopters.

29 CFR 1910.132(a):   Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:              In the alternative:  29 CFR 1910.23(c)(1):   Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:   Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  a) Main Production hangar - On or about December 18, 2012, employees worked in and around the engine and transmission compartment(s) of Sikorsky H-3 helicopters at a height approximately 10 feet above the floor. Work was conducted both on top of the helicopter and from the engine/transmission cover platforms without employees utilizing fall protection or being protected from falling by use of guardrails.
