29 CFR 1926.451(e)(1):     A safe means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders or attachable ladders was not provided.     a)  1718 North 42nd Street, Philadelphia, PA-  There was no ladder, integral ladder, stairs, or safe access access from the structure provided.  Employees accessed the working level at the top of the scaffold over 32 feet high by climbing the frames of the mason's walk through scaffold.  Condition noted on December 19, 2012

29 CFR 1926.451(f)(6):     Scaffolds were erected and used without having the required clearances of 10 feet between any conductive materials handled on them and exposed energized power lines.    a)  1718 North 42nd Street, Philadelphia, PA- Employees were exposed to shock and electrocution hazards while working on a scaffold within 10 feet of energized power lines. Employees were handling shaped aluminum flashing/capping sections that were approximately 10 feet in length on the top working level of a tubular steel frame scaffold.  The upper steel frame was 20 inches from the closest phase of the 13.2KV distribution service and the planked working level was 28 inches away.  The planks were at the same level as the power line. 29 CFR 1926.451(f)(7):     Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:    a) 1718 North 42nd Street, Philadelphia, PA- On December 18, 2012 employees erected a tubular steel frame scaffold on the front sidewalk.  There was no competent person qualified in scaffold erection supervising this work. The upper steel frame was 20 inches from the closest phase of the 13.2KV distribution service and the planked working level was 28 inches away and at the same level. Work began from the top level over 32 feet above the sidewalk below and no standard guardrails were installed or other fall protection provided utilized.  Conditions noted on December 19, 2012.    NOTE:  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.454(a)(1):     The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.     a) 1718 North 42nd Street, Philadelphia, PA-  The workers were not provided training regarding the nature of electrical hazards and fall hazards presented while working on scaffolds.  On December 19, 2012 employees installed aluminum flashing/capping from the top level of a tubular steel scaffold, over 32 feet above the sidewalk below and no guardrails were installed or other fall protection was provided and the scaffold was too close to 13.2KV electrical distribution wires. Conditions noted on December 19, 2012.      NOTE:  Abatement certification and documentation required within 10 days after abatement date. The employees performing the work were not trained by a competent person to recognize the hazards associated with the erection of the scaffold. The upper steel frame was 20 inches from the closest phase of the 13.2KV distribution service and the planked working level was 28 inches away and at the same level.
