29 CFR 1910.212(a)(3)(ii):     Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a)  Employees were exposed to amputation hazards while working on the MAAC and Sully thermoforming machines and the employer did not ensure that the point of operations was guarded.      (b)  Employees were exposed to amputation hazards while working on the John Brown Plastic Machine (30/30 Machine), Model #SR 3030 and Serial #12052, and the employer did not ensure that the point of operation was properly guarded.
