29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety nets systems, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:      (a)	M. Lengacher Son Construction, 11104 Notestine Road, Grabill, Indiana:  On or about December 18, 2012, at the work site at 302 N. Main Street, Payne, Ohio, the employer did not ensure that employees were protected from fall hazards while installing siding on a residential type structure.  The employees were exposed to falls up to 9 feet to the ground.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      (a)	M. Lengacher Son Construction, 11104 Notestine Road, Grabill, Indiana:  On or about December 18, 2012, at the work site at 302 N. Main Street, Payne, Ohio, the employer did not ensure that employees were trained in the proper application of fall protection systems while installing siding on a residential-style structure.  Employees were not aware of fall protection requirements and methods to use to safely perform elevated work.

29 CFR 1926.1053(b)(4):  Ladders were not used only for purposes for which they were designed:      (a)	 M. Lengacher Son Construction, 11104 Notestine Road, Grabill, Indiana:  On or about December 18, 2012, at the work site at 302 N. Main Street, Payne, Ohio, the employer did not ensure that a stepladder was used as it was designed to be used.  The stepladder was used as non self-supporting, single ladder which exposed employees to fall hazards.
