29 CFR 1926.252(a):    Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, was not used:    (a) Jobsite at 218 South St.,  Holyoke, MA: On or about December 20, 2012, employees had been throwing/dropping debris off this roof more than 20 feet to the ground below without a chute as required by this subpart.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) Jobsite at 218 South St., Holyoke, MA:     On or about December 20, 2012, an employee was working on this residential roof more than 20 feet above the gorund below without being protected from falling as required by this subpart:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
