29 CFR 1910.212(a)(3)(ii):     Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a) On or about 12/26/12 in the metal cutting area; the guard on the inlet side of the Stud Chopper ( model # E-10 ) had an opening of three and a half inches with a distance of five inches from the point of operation exposing the employee(s) to the cutting blade of the machine.
